Official list of documents for enrollment in PEC-G/UFJF program

1. Registration Form (.doc format, size 39 KB)
2. Certificate of Conclusion of Secondary Education (2nd Degree), if in a language other than Portuguese, a sworn or certified translation by a competent professional is also required.
3. High school transcript (2nd grade), if in a language other than Portuguese, a sworn or certified translation by a competent professional is also required.
4. Birth certificate, if in a language other than Portuguese, a sworn or certified translation by a competent professional is also required.
5. Letter of introduction for PEC-G – issued by the MRE / DCE
6. Term of Commitment for PEC-G registration – issued by the MRE / DCE
7. Financial Responsibility Term – issued by the MRE / DCE
8. Declaration of proof of income or financial solvency of the responsible sponsor
9. PEC-G student sponsor’s identity document (scanned and copied)
10. Passport: 05 copies of the first sheets and page showing VITEM-IV MULTIPLAS
11. National Registration Card or Application Protocol (until issue of the card)
12. C.P.F.
13. Proof of residence in the country of origin (recent telephone or water or energy bill)
14. CELPE – Bras proficiency certificate
15. Vaccination card – (request yellow fever vaccine to enter Brazil)
16. 01 3 × 4 photo

NOTES:

The original documents, except for the item 9, as well as documents referring to regular entry and stay in Brazil, must be presented to this Office for conference, accompanied by a copy of each, when the presential classes are back, on a date to be informed to the student by e-mail.

After settling in a permanent residence in Brazil, please present the following original documents, accompanied by a copy of each one:

1. Rental contract
2. Proof of residence (water bill – energy or telephone / internet)
3. S.U.S card (mandatory)
